I'm not sure that Omnipresence and Time Paradox immunity can neg Outline Origin given Outline Origin removes every plot twist and reverts the user back to his Base form at the beginning of the story. Only counter is being acausal because them having no past or future.
The Universes in recreators are created by human but once they are created, they are separated from the main world and it's cause and effect and exists as a parallel universe. Meteora explains it in Episode 2. Altair, however doesn't belong to any Universe as she has no backstory and setsuna was killed before she could get one. Which would explain why she is different than the rest of the creations.
