From what I'm seeing his injury was extended to middle of the chest in WSJ while Volume rel has his injury on right side when Fallon which doesn't make sense when the injury was on his left side of the chest?

No, it's not. Itadori when falling turned upside down, making you have this feeling

If you just rotate the body that part that the vol has the wound is the right side, we see Yuji got hit on the left. They somehow got that wrong, likely not even Gege's doing.
