the bullets kill the timelines as soon as the bullet is fired, so Kars dies regardless. With all the timelines where he lives dead, the bullet will find his heart regardless
Ashen's bullet eliminates all wrong futures, keyword being future. It doesn't affect the past, nor will it make its target not exist in the past. Also, the bullet kills only its target. As his Stands can live independently from him, they remain. KQU uses BtD go go back to the past, when Kars was still alive. Since D4C wasn't involved, Kars likely doesn't "understand", but he guesses something is wrong since the effect of BtD can be felt. Fast forward to the future and Kars is facing Ashen again. Except, he knows something's up. So he takes action against it. If he fails, the entire situation repeats again, but he'll be even more serious. Ultimately, he'll do a TWU+KQU/D4CU combo and Ashen is dead.
