Outworld Devourer from Dota 2 has an ability called Astral Imprisonment where he sends them into an astral prison to make his enemy realize their inferior intelligence before returning them to the real world with their sanity shattered.

Could possibly be 1, but the prison itself would definitely be 4. I assume this wouldn't work on stuff smarter than Outworld Devourer though.

It's not 1.

1 would be something along the lines of Johan Liebert talking people into a psychological corner until they go insane.

This seems more akin to Randall Flagg projecting within a victim's mind the birth of the universe inside of an atom, albeit instead of sending someone a vision, this person BFRs somewhere.

He did say that they're driven insane through the forced realization of their inferiority.

OD doesn't interact with the opponent at all, he just leaves them there to contemplate on their inferiority before returning back.

We could say that the guy literally sends you to another plane and you are entraped. About the type of madness... there are a few things that lead me to belive about the 1 but type 4 seems like the most logical.
