Because Dream of the Endless is potentially superior to Mister Mxyzptlk, as the ruler of the Dreaming, a place of imagination and story. This is the reason for the High 1-C tier.

If that's the case, wouldn't the Thought Robot and Mandrakk be a little higher since one of them is considered half of the extension of the Overvoid?

None of them are half. All contradictions are infinitesimal to it.

A probe/extension doesn't necessarily have the same power as the Overvoid, so there's no direct scaling.

The 5th dimension seems to be the imagination of the creator/perpetua

The Speed Force should be Low 1-C since it's above the Bleed, right? Wally, Barry and Hunter should be opened as well.

Off-topic but the Sixth Dimension was described as a realm "out of time" by Batman: "We're dealing with a realm out of time. Who knows how long he'll be away? It could be months, years, decad--"

Perpetua is a maybe but I don't think so for the Monitor Brothers. The realm is what's being specified here and all the Brothers are rent from the Overvoid. Perpetua, at least, is a Supercelestial and should be outside the normal time.
