What's koku's ap and what can Killua do against the dudes regeneration?

Koku upscales from 70.1 Tons. And killua has that nichirin sword and he knows that if he cuts off koku's head he could win. Depends on if you think koku would evolve or not

Nen crush is when Killua releases his Nen, and causes both fear and madness manip. Though, koku has an aura of his own, so there's a slight possibility it might not work.

Regardless, what can Koku do against Killua's higher ap advantage? He has a plus sign next to his rating, which means he is at least >550 tons.

What really stops Killua from just cutting off Koku's head, really?

Killua's nen crush at this point should be far above to the likes of Wing who can make normal person completely terrified, then the kid also scared Rammot who is able to use Ten.

Regeneration is moot because Nen attack the mind and Koku cannot defend against it.

Putting aside the Nen crush stuff. Killua massively scales above 109 tons, while Kokushibo scales above 100 tons so the boy got AP advantage here. However, Kokushibo are incredibly skilled and he also resist regen null.
