Can someone explain (in the least painful way possible) all of Naruto's forms?

I haven't kept up with the seies since halfway through Shippuden and I've lost track of Naruto's forms and techniques. Could someone do a rundown of all his forms (kyubi, sage etc.) from pre-time skip to now so I can catch up? I tried looking online but it's all over the place and it just gives me a headache so I'm still confused. Thank you!

Sage mode - used first to fight Pain (Nagato) . same as Jiraiya but mastered (Jiraiya not complete). Kabuto later use sage mode also (but snake form rather than frog) to fight Itachi and Sasuke.

Kyuubi Chakra mode - obtained after train with kilerbee in island. Naruto overpowerd kyuubi and took its energy. used first in team with killerbee to fight Itachi and Nagato. this the fight were Itachi breaks edo tensei and join good guys again.

Bijuu mode - Used first to fight Tobi(Obito). Kyuubi is fully support Naruto now, and friend with him. this form is Naruto + Kyuubi full power together

SIx path sage mode - Used against madara. Naruto meet hagoromo (the first sage) and receive his power

Kyubi forms - Naruto takes the form of kyuubi. this can be used in almost all modes. dpending which, naruto will lose control or remain in control. (like after becoming friends with kyuubi he can take its form and stay in control)
