All main characters should have Type 8 Immortality-

As long as there is a nearby hospital, you will always come back to life.

Wouldn't that mean its just the hospital and not the characters

You're right :^) perfect probably ressurection instead of immortality. If we're using composite , it appears you go to heaven when you die the first time in GTA online :^)

It likely applies only to the Gta online protagonist, for the others not because it happened that a main character totally dies during the story, one example is Gta V end where you kill one of the protagonists and obliviously doesn't resurrect.

For plot purposes that is true, but if you kill one of the 3 protagonists in story mode, they send you a text a few minutes later saying they're sending you the hospital bill.
