The Sleepers from Adventure Time are likely the weakest Low 2-Cs, given their ability to dream entire universes into existence likely isn't combat applicable, and they can be huge Glass Cannons

Weakest 2-A character is probably the Forerunners. Their 2-A rating isn't really combat applicable, they're probably gonna get downgraded to Low 2-C because that's their actual best destructive capacity, and they were defeated by computer hacking.

About the weakest High 2-A, at least according to this wiki, seems to be Wyrm from TMNT 2012: he is 5D only because he was in a 5th dimensional prison, and has feats that would be common for a 4-A, and while unselaed, he would have brought chaos to the Universe, something that any 3-A could do.
