considering what was explained mainly in the Japanese version of the Pokémon Platinum event it can be said that this is a reference to the original state of the Heart before the world was made, what is mostly called the “Original One” in the English localization.

If you want the logic on why it went the way it did:
Alright. That makes sense.1. The game only ever says "プレート" (Plate), since Japanese lacks a proper plural there's no way to know if they want to say about a single plate or all of them.
2. Same thing, they all just say "プレート" (Plate) and that is how it's always mentioned, it's impossible to know how if it should be singular or plural just by "looking the Japanese text.
3. It says "the power of the plates are shared with the Pokémon".
生まれてくる (Born) ポケモン (Pokémon) プレートの力 (Plate's power) 分け与えられる (Shared/distributed)
