Its no that easy as you think. Koishi can keep people from being aware of her and, by extension, her location. Not knowing where Koishi was would presumably have kept Flan from getting a grasp (pun intended) on the location of her 'eye' as well.´╗┐ If thats not enought she could even hide her existence.

Although there's also the whole children are immune to Koishi's powers (And Flan is often noted for being childish, moreso than Remilia), Flandre is not only at least 495, her mind is more mature than say, a 12 year old, one instance of this being in that time when she destroyed an incoming meteorite, and talked about her ability a bit.

Koishi can stay invisible even in Flandre's plain sight using her power and start messing with her subconscious with her mindhax. Flan won't know what hit her; Koishi victory. (Having no prior knowledge gives the green satori more of an advantage too, she can conceal her existence from the sister of the devil, rather than just her presence)

Although... with the current profile's stats, Island-level AP vs Large-Planet Durability, this battle will take a hell of a long time. So much so that their stamina would probably run out before the battle would end.
