"Obviously the misery they're referring to must be disease" aren't they literally trapped in the Greek equivalent of Hell. That also sounds miserable, if I'm being honest. Possibly moreso than being sick.

The arms... aren't trapped there. This applies when they manifest in the mortal plane as well. And also applies to other lost souls they grab who are already in Hades so that doesn't make much sense.
 
the arms are considered trapped there since they are commonly found in hades, but they can manifest into the mortal world to drag mortals into hell, so they aren’t trapped in the sense that they can t leave.( they are extension of hades since well they are his ^arms^) but agree to disagree.

In my opinion, sharing their misery seems more likely to refer to the thing we know they can do (drag people to Hell) and not something that isn't as clear cut (they're diseased, so naturally they share that disease with other things). I suppose trapped was the wrong word, but the point is that dragging people to Hell would in fact make them miserable.
