Exactly what it says in the title, how do things change going forwards? To make things a bit more interesting I'll split this into three rounds.

Round 1: It manifests the day after they're born or at the very least whenever they can function as a person.

Round 2: They gain it on the day their debut moment in the story takes place, whether that be before they actually become the protagonist yet or not. (Example: Izuku as a little kid or Naruto during his flashbacks as a little kid)

Round 3: They gain it the day the story proper starts since some series introduce a character way before they can do anything of value in the series. (Example: Izuku during the first chapter in middle school or Shirou in Fate/Stay Night since technically his canonical debut was in Fate/Zero)

DMC first round and second round:

Sparda would study the wheel during Dante's 8 years of life, Vergil would probably want one

third round:

Dante would be confused but would probably think it was something related to his demonic side, his adaptation would skyrocket, but it would be a hassle for him to walk outside with the wheel in the first few days if he is not on a mission.

Doctor Who all rounds:

Great now there is a race of Mahoraga (Thanks to Timeless Child, we know that the Time Lords in some version of history experimented on the doctor, even if we go back to another version, the doctor was previously The Other, the third founder of time lord empire, so they would investigate the wheel), what do we do when the Daleks copy it during the Last Great Time War? I mean both sides (time lords vs Daleks) would incon but in the end it will be the same

now after Time War, the Doctor's adventures would be easier, the Daleks would leave the wheel aside because that would go against the idea of keeping the race pure

Also anyone could say that the Doctor's conflicting timeline is because of the wheel, which solves a lot of problems that the fandom has lol
