No, he NEVER uses the Heart of the TARDIS in Character. The only reason we even know about it is because Rose used it, and using the Heart of the TARDIS kills the Doctor.
He'll have to Regenerate, which he does not like doing. Doctor just wouldn't Atomize people, even if he's willing to kill (which is often), he's more likely to punish or defeat them.

After seeing Oudo use it once, she learned it and mastered it better than he did, despite having it at 100% efficiency via Unreasonable Taxation.
For the Doctor, she'd be able to learn pretty much all his innate abilities because as the Doctor himself says, he talks too much.
But I don't really see Medaka doing anything to the Doctor, not because he resists or he would stomp her out, but why would they even fight each other in the first place?
If we assume the Doctor is going absolutely insane like in the Doctor Who Comic where he joins the Baddies and tries to **** up the timeline, then Medaka would just keep trying to Reform him, and considering Clara could do that by just putting on a sad face, I have no doubt that Medaka would reform him.
Like, she can reform a Minus, who's entire existence is just being a Major L is every category. The Doctor wouldn't be that hard to reform, and once done, they'd not want to fight each other.
So, I'd ultimately go with Inconclusive, but if assuming a Doctor who would want to attack Medaka, then Medaka would reform and then Incap (?).
