I have no idea how to start these. So let us start with his tier.
It was brought up that Mom's size is way bigger than we think. Her size is kind of inconsistent, however she is always shown large regardless. We can see Isaac is the size of her eye. Even if he's a small child, that should still be rather large. Her womb alone contains several rooms as well as her utero. I think Isaac should be upgraded to 8-C, or possibly higher due to the scalling chain from several stronger enemies and his items.
Isaac can also have a potential speed upgrade. He should be able to dodge his own bomb explosions and can keep up with enemies who explode when dying. Depending what items he has, you might need to fight close range. This might need to be calced.
Now for some abilities. These mostly come from Isaac's and ???'s boss fight. These include:
Light Manipulation: Makes beams of light to damage people.
Flight: Can grow wings to fly.
Teleportation: He has this already with the Fool card, however this time he can control where to go and it isn't a one time use.
Homing Attack: ???'s boss fight will sometimes use homing tears.
Summoning: Can summon flys that shoot blood and angelic babies to attack.
