Inherently Shockwaves aren't dura neg with no feats, it's only when you pull something like Whitebeard or Garou can do that they become dura neg.

Yes, it seems shockwaves in One Piece always hit the inside and make you cough blood.

IMO Blackbeard shouldn't really scale to Garp (especially since it was implied that Garp could kill Akainu), but he did match Sengoku yet was scared of Akainu.

BB is an opportunist.
He probably thought it'd be too risky especially when Akainu probably would have taken down a good amount of his crewmates even if BB won.
After all he ran from Old Rayleigh who should at the VERY least downscale from the Admirals, even though he's done things like blocking a slash from S-Hawk who matched Buso Zoro.
