How are creation based feats handled?

Since (to my knowledge) it's impossible to make something from nothing in The Real World what's the general consensus on creation feats? Is it just considered the same as the equivalent energy output of completely destroying the object being made unless stated otherwise? I'm curious because sometimes feats seem to make the character seem really powerful. But then again I have a hard time mentally conceptualizing some feats in fiction.
 
We normally assume creating something at least the size of the moon is reflected as the corresponding AP unless we have a reason to believe otherwise.
