Does John have a wheelchair? I know speed is equalized, but if he can't stand up, that could still be a disadvantage.

You said he's close to death. Do you mean like at the point of Saw 3 where he was in a hospital bed and needed surgery? If so, Burns should have more stamina than him and could potentially just survive until the cancer killed John.

Not close to death. The closer point to the end of his life. When he was becoming to ill to move on his feet and before he was killed. Meaning he still has a while left in him. (Least more than a week)

But that reason is still valid as Mr Burns longevity

think burns wins from just waiting out the cancer to kill him like what frieza said but he can also keep his range with the baseball bat as a poking stick of some sort and play the waiting game until he dies
