From the anime flashback, Yamamoto straight up killed him with his bankai slash. And according to the legend... The Confined Quincy King poem makes it seem like Yhwach effectively did die. "The Confined Quincy King regains his pulse after 900 years..." pretty strongly implies Yhwach was straight up clinically dead for 900 years after his battle against Yamamoto and the OG Gotei. Ichibei said Yhwach would regain "Almighty" the moment he died. Yhwach could had use his Almighty to bring himself back to life? Because the other part of the poem says that if he is to open his eyes before time he would robe all Quincies of their powers. He had Almighty since Yamamoto killed him but he never used it until the Ichibei fight.
