Do 1-A locations have to transcend matter?

Theoretically yes, under the definition of physics we use today and the classical philosophical definition, but it definitely depends on how physicality and matter are approached in any given setting. Some settings have pseudo-physics on the highest levels which are given a pass due to just being approximations which hit the right buzzwords (Xeelee, Marvel, etc.)
 
99.9% of the time, yes, unless the verse does some real convoluted work to make them reach that level without involving any normal physics.
I don't know of an example which exists, but I guess it could be theoretically possible.
 
Unless you're a Chinese/Cultivation verse where usually the higher power source grants "access" to the lower realms, yes. They have to be completely and utterly qualitatively superior to anything "below" them. I know exactly why this thread exists though and it's funny when I asked this I never got an answer
