2. Common sense says Kefla didn't defuse because they're stated to be in a place without time or space, or the fact that the fight only lasted about a minute, though if Kefla is infinitely stronger than Vegito then the former applies
3. Goku's forms all get stronger after UI. Kale forced him to go SSB when he could've used SSG (Whis states SSB maximizes speed and power). Goku while weakened used SSG and defeated an improved Kale and Caulifla at the same time. Or there's the fact that he could keep up with SSJ2 Caulifa and blitz her in base before she adapted.

Time is either in effect for everything, including the time limit. Because reminder, if time wasn't in effect for the enture arena then we'd get infinite everybody.
