If he has a heart and is alive (i.e his heart beats), Murasame will kill him. There's no indication it spreads through the blood. If anything, the manga shows it spreads through the curse markings that appear on the wound.
On average, a blood cell needs ~1 minute to make a round trip to the heart. If we assume half of that time (since Esdeath was sliced on her arm therefore the time it would take the blood from her arm to her heart would be shorter), that gives us ~30 seconds. Since she was fighting, her blood flow would normally be faster, so assume half of that time, meaning 15 seconds.
With 15 seconds to spare (which is a lot considering Esdeath's speed) she wouldn't really need to use such an exhausting move like Mahapadma to slice her arm off.
Another proof is Akame slicing fodders with Murasame and the curse activating almost immediately after being cut.
This is why when you're bitten by a venomous snake it needs a while for you to feel the effects of the venom and it is advised you do not move around to much because the venom spreads faster that way.
