That said, the enemy here is Regulus, I'm not convinced that SBA would make Tatsumaki instantly kill his wives. Unless you could explain your reasoning more clearly.
Knowledge of the other character/verse: The characters know the direction their opponent(s) start in. Furthermore, they get knowledge of the character's appearance, equivalent to the knowledge they could have gained if they would have been allowed to briefly observe the form opponent(s) starting in using their senses of sight, hearing, smell, taste and touch (or equivalent). It's assumed that they only get the information necessary to identify the opponent(s), so abilities like Information Analysis can not be used based on this knowledge. Negative effects of the observation, such as those caused by Madness Manipulation Type 3, are assumed to not apply for this knowledge either. Aside from this, the characters have no knowledge of each other.
Tatsumaki knows that Regulus and the wives are her enemies

If Fubuki didn't warn her that her subordinates were being controlled, Tatsumaki would have killed them alongside Do-S. The same thing would happen here except the wives are enemies so she wouldn't hold back.
It is already in Tatsumaki's personality to do so.

Regulus’s heart being missing does not give off any energy.
Regulus's heart would still be something alive that emits some kind of energy like everything alive and can still be detected, obviously Tatsu would find only if she interacts with his wives and that would happen as she begins to destroy the city and move them around to save them, the energy does not come from Regulus, comes from the cancer heart inside of one of his wives, it is not like a pill specifically hidden inside of someone's body, it is a heart

there is no "cancer heart" btw, its just a regular heart and people having certain conditions or abnormalities within their heart would not rise enough suspicion for her to simply crush part of a random wife's heart
 
It does not matter if they are next to Regulus or not. Other than Regulus and Tatsumaki herself, the wives are the only other people in the area, and they are still well within Tatsumaki's range of Thousands of Kilometers. I have explained above why because of SBA Tatsumaki sees them as genuine enemies and I have shown above how she treats enemies. She does not withhold lethal force at all.

She does not need to find anything out. SBA states that knows them as enemies and no matter where they are within the 100 kms, they are still within Tatsumaki's range. Tatsumaki does not need to individually crush the hearts of the wives. She can either nuke the entire area or just erase them from existence. These are her starting moves.

Tatsumaki has shown the ability to avoid extreme homing danmaku and can switch out with decoys whenever the match starts she would fly off at faster speeds than Regulus can react and kill the wives as he only has normal human reactions.

Just because Tatsumaki can perceive the energy of living things doesn’t mean she can sense internal organs or biological anomalies like a missing heart. Show me a scan where she’s ever detected something like that. Sensing living things won’t help her figure out that Regulus’s heart is missing or that it’s inside his wives. She doesn’t have any abilities shown that would let her sense this kind of specific biological detail.

She has no reason to look inside Regulus’s wives for a heart because she wouldn’t even know that Regulus is missing one in the first place. You’re assuming she’d randomly decide to scan for a foreign presence inside his wives with no reason to do so. That makes no sense.

Tatsumaki needed a direct hint from Apollo before even checking for the telekinetic energy inside Fubuki. She didn’t just instinctively sense it. If she required that level of guidance to find a pill specifically reacting to telekinetic signals, what makes you think she’s going to randomly check Regulus’s wives for a second heart with zero prior knowledge?

It isnt in character for her to kill anybody despite whatever you seem to believe, so i actually see her holding back so she doesnt kill the wives accidentally to be more likely lmao

The fact that she will find the heart he hides in someone else is for the fact that Tatsumaki can see the energy coming from one being and with that distinguish one person from the other, even if she will see only a sphere, as she looks at the wives to protect them from the destruction she will see two spheres from one of them. Tatsumaki can see the flow of energy inside of ones being, like she did with psykorochi, so I don't see why she wouldn't be able to do the same with them

You still haven’t shown a single instance of Tatsumaki sensing organic things as energy, only external forces or objects linked to energy, like Apollo’s telekinetic signal.

When she entered Psykorochi’s body, she was surprised that her chest was hollow, which proves she can’t naturally sense internal structures. If she could, she would have already known before entering.

There’s nothing in her fight with Psykorochi that suggests she could detect Little King, and she wouldn’t just “see two spheres” because a wife has two hearts, the spheres don't duplicate themselves based on how many hearts you have. If that were the case, Reinhard, who can detect life, would have sensed Regulus by looking at Sylphy and see two "life forces". Emilia, who can sense life force and presence, would have noticed two "life forces" or "presences" in Sylphy since she was around her for a long time. Neither of them did.

she also only knew Psyko fused with Orochi because she physically saw them fuse, not because she sensed anything hidden inside, besides, nothing in that chapter shows her sensing them fused, not like it would matter anyway
 
Nothing in that scan shows anyone actually sensing internal body parts or sensing anything, just ki flowing through their own body. Even if they could sense their own ki pathways or other people's ki pathways, that wouldn’t help locate Little King, because the wives aren’t infusing any ki into themselves.
Everyone has ki? What you're seeing in the image is someone simply extending the ki already in his body to his blade.

How would that help them figure out Little King, which is merged with the wife's heart? They would just see the same ki flow as any other living being. Nothing in that panel actually highlights specific organs like the heart or brain, so at best, they’d just see a continuous flow of ki, not something that tells them there’s an extra foreign organ inside, since it's merged.

Let me explain.

The OP states that Regulus has 2 wives next to him while the others are outside of the city he's fighting in. Because of SBA, the wives both inside and outside the city, Regulus, and Tatsumaki are the only people here. As I explained, the wives near Tatsumaki would get killed in her attack (Regulus would survive due to his ability) and it's been said in this thread that Regulus tries to protect his wives. This would allow Tatsumaki to draw a connection between them and him. They would then be considered as genuine enemies. If the OP didn't have Regulus have a couple wives next to him to start then I could see what you're saying but that's not the case.

Tatsumaki's flight and attack speed are far faster than Regulus' reactions and 100km is still within Tatsumaki's range. Like I said above it's instant death for the wives from there.

Tatsumaki herself can avoid getting attacked while doing so as she's shown danmaku dodging feats and she can also create several decoys to prevent taking hits. She can do all this while attacking at the same time since multi-tasking is bread and butter.

Tatsumaki can sense spatial distortions so she'd know how to avoid. Tatsumaki has far higher general speed meaning she's going throw out her attacks far before Regulus can 10 times out of 10.

Because she can sense spatial distortions, as soon as the battle starts she'd be wary of Regulus and would know that her attacks may not work. From that point, from what I've heard Regulus would be more likely to as a result try to protect the wives rater than attack as first move. By the time he actually attacks Tatsumaki would have swapped with a decoy and destroyed the rest of his wives with the twist of her fingers.

He doesn't have to aim accurately since he can use large scale AoE attacks, and the attacks being invisible also doesn't matter when she can't react to them in the first place. Regulus being a distortion in space would make her cautious, but she has no way to even predict his attacks and no reason to assume his attacks are FTL instant kills. On top of that, he generally doesn't give any clues he's going to attack. He doesn't need to perform any gesture to attack, just breathing alone is enough to make him win this match.

Just realized but is Tatsumaki very mobile during her fights in the first place?

Regulus isn't likely to attack first, and would yap for a while before doing anything. Tatsumaki will have nothing to dodge, so I doubt she'd be very mobile. Often times she just stands (or floats ig) around and start by crushing her opponents, right?
