Reality-Fiction differences

Why do reality-fiction differences have to do with the tier of a character? Reality-fiction differences are only relative. If reality-fiction differences is considered into a character's tier, then any character smart enough to imagine a Tier 0 would instantly become a Tier 0 themselves, because they vastly transcend a Tier 0 in their imagination.
 
Say there is a weird cosmology, where above everything else, exist entities similar to the Monarch of Pointland. They are just points. They can't do anything except for thinking. However, they are so incredibly smart that in their imagination, exist characters who themselves imagined another realm which contained characters who imagined yet another realm and so on to infinity, and each realm have stronger characters than the character who imagined that realm. Logically, the power of each realm go up as we go down the imaginations within imaginations. But considering reality-fiction differences, the "monarch of pointlands" will be tier 0, and the power decreases as we go down the imaginations within imaginations.
