The brain things a bit different, cancer cells can be hard to detect in patients and there's also the fact we're asking sorcerers to be nearly as precise as Gojo with their control of ce to target the cells. People just forget disease and biological stuff still apply even if a character is tier 6 with ftl reaction speeds and super cool abilities.

I could see Gege developing it to be brain cancer brought on by something unique to ce maybe.
