Ragna the Bloodedge and Hazama/Yüki Terumi Perception Manipulation, Mind Manipulation and Memory Manipulation - Coming into contact with the Azure caused everything going black for her. It is also confirmed that the Azure/Boundary causes one's conciousness to be shattered and also causes people to lose their memories.

Ragna made it so that people would see him as a hero, so he got severely weakened there after because there was no hate for him. And probably plot

Ragna planned for him and Yuki to be sent/slayed into the Blue by Jin wearing the Susano'o Unit. Thanks to the Master Unit seeing that, it recognized the Susano'o Unit (therefore Susano'o) as the one who slayed the ultimate evil (which was Ragna in the timeline). Because they hailed Susano'o as a hero instead of being feared, Terumi's power was MUCH weaker and due to them being in the Blue (and therefore the Boundary), Terumi was at a genuine risk of being killed. Ragna was strong enough to the point to use Black Onslaught on him and wipe his soul completely, ending him.
