And given how it's listed on his profile, it's a wish he has to make, not a default thing, passive > having to wish for it

Pretty sure Timmy is just a normal kid most of the time and needs to make a wish to get any weird shit. So, unless Timmy gets perp time, I don't see him even getting a incon.

The Almighty is passive, Yhwach will see the future and make Timmy's wishes useless.
Since it's something that has to be wished for, he'll get soul crushed before he can say "I wish". And considering he has no knowledge of what Yhwach is capable of, he'd have no reason to wish for Inorganic Physiology at the start of the match.
