OPM: Few Additions

The biggest issue with that is Awakened Cockroach is off-screen prior to us seeing him be hit. We only see the aftermath; we don't see what he was doing before getting hit.

Maybe he started moving for all we know but could only make it an inch before getting pierced; him being surprised can simply be attributed to being surprised Orochi is so fast or being surprised that there's a massive horn stabbing him in the chest and his realization caught up to his instincts.
Now that would be an assumption.

Other than the fact that this would be shown to us if that were the case, his surprise isn't "How?" or about Orochi's strength which is very clear.

He says "What are you doing?". He quite literally questions the intent which is dumb as he's supposed to know about it.
Garou did dodge Orochi's horn. It was the follow-up attack that hit him.
Yes. Garou dodges an attack he sees that is coming, not about instinct. The surprise part is the follow up where the horns are flexible.

That too is an assumption.

Since there is no way to know for certain, the best solution is not to make additional assumptions which is that Orochi has some unexplained ability to negate other people's instincts.

Dumb writing? Color me shocked. It could be as simple as an expression of disbelief. After all, he says his question after he has already been stabbed. Wouldn't it be apparent to him by the time he spoke that he's been stabbed making the question redundant if it was genuine?

If i see a character flying, i'd say they can fly without saying it's not explained. I don't think this would count as an assumption to the point it wouldn't be acceptable.

What we're shown is that Awakened Cockroach doesn't do any type of action before Orochi starts his attack. Which is clearly shouldn't happen based on his ability.

The anime version also shows it to be the case that Awakened Cockroach didn't do any type of action. Orochi looks at him (even the direction of his eyes doesn't change) and attacks with a linear attack to the same direction he was looking.

Also the fact that he says "What are you doing" while being surprised, which doesn't make any sense as he senses the intent.

Yes, he asks his question after he's stabbed but not sure how it supports anything you said. Was he supposed to question it before he was attacked? He would only do it if he were actually sensing Orochi's intent, absolutely no reason for otherwise.

Btw, it could said to be dumb writing if Awakened Cockroach tried to escape or anything. But everything he did completely suits the situation.

My claim isn't an assumption, it would be an assumption if i were to claim it works in some specific way or so. Orochi does ignore it, i don't know what he does, completely unaware of the method.

But it is irrelevant in this case as he does show us that he bypasses Awakened Cockroach's ability.

Assumption would be that "maybe he tried to escape which it wasn't shown or even implied in any way"

Even at the very least, as the method is unknown, it should still apply for "Possibly" as it is shown to be the case here.
