Because the rules are that you don't get the "+" without a calc.

Madoka doesn't have a 5-B+ calc, just like Ryuko doesn't have a 7-A+ calc, so they don't get the "+". It's simple as that.

Either way, IMO "+" for Madoka is more accurate, because it's closer to how strong she actually is. Don't know about Ryuko, but I'm fine with it being removed.

Senketsu is also super skimpy for the pure purpose of reducing how insane it makes the user.

Junketsu actually is build to make the wearer submit, but Satsuki kinda just said "fork you" to it as she wore it.

Junketsu and Senketsu are so skimpy because Kamuis have to minimize skin contact with their wearers to prevent damaging them due to being made entirely of life fibers. Thats canonically the reason why.

Anyways, the scaling chain is Mk 2 Elite 4 (Tanked 104 Megatons with little issue) => COVERs and Nui Clones (Able to overpower the Mk 2 Elite 4 with ease) => Combat COVERs (Exponentially stronger than normal COVERs and was only staggered by New Fight Club Mako, who can casually oneshot normal COVERs and Nui Clones) => Mk 3 Elite 4 (Able to casually oneshot dozens of COVERs with indirect attacks, Gamagori in particular being able to casually oneshot a Combat COVERs, as well as oneshot Nui clones) => Nui (Casually decimated all four of the Mk 2 Elite 4 simultaneously. Able to fight on par with TLFS Ryuko for most of the series, but was ultimately surpassed when Ryuko got her zenkai from tearing off Junketsu) => Satsuki and Ryuko (Stronger than the combined power of the Mk 3 Elite 4, stronger than Nui, and able to stalemate each other) => Ragyo (Fought Ryuko and Satsuki simultaneously so casually that she wasnt even paying attention to them and had her eyes closed, then proceeded to oneshot both of them as soon as she got serious, as well as having oneshot both of them multiple times throughout the series)

Simplified:

• M2E4 - 105 Megatons with little to no damage
• Covers - Casually stronger than 105 Megatons
• Combat Covers - Exponentially stronger than Covers
• M3E4 - Casually oneshot Combat Covers
• Nui - Slightly weaker than Ryuko and Satsuki
• Ryuko and Satsuki - Stronger than the combined power of the M3E4
• Ragyo - Immensely casually stronger than the combined power of Ryuko and Satsuki and has oneshot them both multiple times

Technically we could also use the number of people contained within the Combat COVERs as a multiplier as well...

I should mention that both Ryuko and Satsuki have moderately injured Ragyo, shouldn't I?

• Satsuki decapitated her when she revealed her revolt
• Ryuko cut her cheek when she faked her death to get to the Primordial LF
• Satsuki impaled both her palms with her 2 Bakuzan blades
• Each wielding a half of the Scissor Blades, they sliced Ragyo into parts right after Senketsu disabled Ragyo's Omnisilk Absolute Domination by separating Hououmaru from Shinra Koketsu

That doesn't warrant 7-A+
