How are one time Transformations cataloged

This is a two part question

1: When cataloguing 1 Time transformations, is the key supposed to be a hypothetical Current Base form + The Transformation or does it only catalog that specific time they were that transformation

2: What’s the preferred key ordering. Base (First Era), Base (Second Era), Transformation or Base (First Era), Transformation, Base (Second Era)

1: When cataloguing 1 Time transformations, is the key supposed to be a hypothetical Current Base form + The Transformation or does it only catalog that specific time they were that transformation

I think only the specific time they were at that transformation, an example is Naruto, who doesn't have the Ashura Mode in his Boruto era profiles (which could be listed as keys but I believe they were separated because it'd be too long of a profile), it's also what makes the most sense, the next key is the character after that, which means after he used the ability, so he shouldn't be able to use it again

I believe it's Base (First Era), Transformation, Base (Second Era), but I'm not so sure on that
