What qualifies for for Type 5, and Type 4 Acausality???

I am seeing some conflicting responses from both Staff, and non-staff on what "Qualifies" a character for Type 4, and Type 5 Acausality.

So, what is the mininum needed to qualify for each for those 2 Type's?
