To be fair they were both bloodlusted there. Here only the capy is bloodlusted. Plus it has slightly higher stamina, pretty smart for an animal which might catch Walt off guard plus there are recorded cases of capybaras successfully killing people so it’s not impossible especially considering in this case it’s a cancer patient.
 
If Walt doesn't get hurt too badly, he can probably grab Sento and kill the Capy with it (I know that it's a magic sword but only when it's wielded by Kenshi).

Ahh I forgot that sento was in cage mansion 💀, feel like I should probably change the location then because Walt would just gut the capy pretty easily with a katana

Staffs shouldn’t be a problem, polearms might be a problem tho. Although considering the capybara is bloodlusted Walt would probably have a hard time keeping to a range where the weapon is most effective.

Considering the capybara is far faster than Walt (and Walt is known for not being able to react to people even when their speed is equal) the capybara could probably filet Walt’s leg like a fish with its 3 inch long teeth likely rendering him unable to stand while the capybara just rips his throat out.

For these reasons I’m more inclined to think the capybara could pull a win more times than not
