The rest are fine, but couldn't this one just be her using x-ray vision to find where to target with her heat vision?

That's true. Though the place she targeted and attacked was inside the enemy's skull, so I think she meets qualification for "the character can directly target the internal organs, thereby ignoring the durability of the outer layers" description on our Dura Neg page

Yes, but she targeted that through enhancing her regular vision with x-ray vision so she could target said spot, there's also smoke emissions from the person she used the heat vision on, which didn't seem like she was targeting internals but rather abusing her ability to target weak points through other aspects of her vision.
