Questions about ghost and how victory by death work

So a character I'm making a profile for, died during the main story and reincarnated as a ghost, she had a few feat in her ghost form.
Everyone in the verse become a ghost after their death and become a ghost right after their death, my first question is should I put something on her profile like:

Reactive evolution: Will become an entity (how the game calls ghosts) right after her death should she lose her life and thus gain the same abilities as her third key (her ghost/entity key).

Also if she is killed in a fight, if it's victory by death does her getting killed end in a defeat for her or will the fight continue as she become a ghost.
