Do you need to transcend an infinitely-layered outerversal hierarchy and any expansions of it to achieve High 1-A? Or do you simply need to be above any extension of it with the hierarchy being, like, 20 layers in?

yes, however proving it would be incredibly difficult.

Another example that may be useful is the Creator from Umineko, from my understanding is "it" is boundless on the basis it has no restrictions when compared to a high 1-A character (Featherine), which effectively implies no such extension of a high 1-A structure would ever surpass the Creator.

The same logic (assuming the context permits) could apply to a 1-A character and a high 1-A character.
 
Basically, High Outerversal beings are incomprehensible to Outerversal beings. It's a level whereby stacking infinite Outerversal hierarchies won't reach it.
 
If it is more transcendent than the 20-tier hierarchy you mentioned, yes, but higher than the 20-tier hierarchy it would only be no.
