Does this Captain Adam include the 60s version as well?

No, it is a new Grant Morrison version of the Captain Atom character (and also a reference to Doctor Manhattan) created for the Multiversity event.
 
He actually first appeared in Final Crisis, and was later expanded during Multiversity.

He's a fusion of Superman, Captain Atom and Doctor Manhattan, intended to be the most powerful superhero in existence.
 
You are correct about Final Crisis, but he is not a fusion of Superman, just of Captain Atom and Doctor Manhattan. It was simply mentioned that he is the ultimate version of the Superman archetype.
 
That is the original Captain Atom.
