Everything except the Instinctive Action seems fine to me.

I simply don't see proper evidence for it in the scan, seems more like an assumption, unless I'm missing something.

Himeno has a contract with the Ghost Devil, she can freely summon/control one of its hands. Because it's a ghost, it can become intangible. The scan just confirmed that she could directly attack internal organs like the heart using that.
