I think the misfortune has to occur first. Anything bad that happens in his sphere he can negate it or redirect it. For example he couldn't cutt kang du because of his iron skin so he negated the misfortune of him not being able cutt him. In his fight with shunsui and nano he negated her anti Quincy barrier. Which was also considered misfortune. When he was going to engage shunsui he said they were equal in power. Then hash said he was tip the scale of power in his favor. I also believe he may have used the balance to cutt ichigos sword in 2 cuz even yhwach who's above him couldn't break it in one blow. So yeah it can work outside of damage the only thing is what is considered misfortune to hash. Cuz it seems he can use it in almost any situation
