About Scan

Do all Abilities need to have a Scan?, or are just a few enough? What are the requirements for a Scan to be included in an Ability?

for obvious abilities you can just give reference. for example if its their powers are just listed in their arsenal you don't need to screenshot it being shown but simply add reference to which chapter it was shown etc usually in game characters.

But abilities that qualifies as a hax you'd need proper scans for the ability to be included or accepted

Lastly supernatural luck and supernatural willpower are unique cases where it needs to be evaluated whether it qualifies or not if their scenarios are not as blatant

And oh, about resistance, I saw that some profiles use scans, some don't, so how do you determine whether a scan is used or not? Specifically for resistance only.

Yes.
You can probably even use just a single description for all those abilities together like
