1. Because Mid-Godly is higher than Low-Godly.

12. A soul is Non-Corporeal. It has no physical form, thus physical destruction tier means nothing. However if said character can effect the soul with their attacks then it's whatever level said character's soul can take.

Not all souls in fiction are the same their is no solid level.

1. Low-Godly = regenerating from complete destruction of the physical body, meaning you are able to regenerate from just your soul/energy/whatever.

Mid-Godly = regenerating from complete destruction of your physical body + the destruction of your soul/energy/whatever

12. Depends. If there is a void after the universe is destroyed pretty sure you need (the current version of) High-Godly, or maybe the ability to exist in a void + Mid-Godly.

How much Mid-Godly can take might change depending on how the Regenerationn revisions go.
