It isn't exactly clear how strong the plane form is, but Drive Knight did already casually dispatch Nyan. Unless the Psykos-Orochi plane is massively weaker than either of their previous forms even when they were separated, there's probably enough evidence to upscale him.
Hundred-Eyes Octopus has a + rating not just because it's pretty close to entering a higher tier, but also because it grows in power when consuming its surroundings, and it was still growing by the time it was killed.

The feat is still the same feat regardless of what the area it happened in is - it just means that the abandoned area is far larger than we might have expected.
Z-City is not completely abandoned - this is explained in Chapter 20 and in the first season of the anime; there's just a large portion of the city that was basically evacuated following the increase in monsters in the area, and fenced off, which became the "ghost town."

Personally think all of the ones getting downgraded should get an "At least low 7-B" rating, given the gap between it and Gouketsu's own rating making it very possible that regular Dragon-levels are more than 4 or 5 times as powerful as Choze.
