For the 1st place: Qebehsenuef - As a joke, they asked a Lot of people to pronounce his name correctly (Nobody could)
For the second: Khi'zar Kha'jul

Btw. I'm not sure how far you are counting such in-universe technicalities, but Inheritance has some dragons with lore-wise unpronounceable names.
First, there are the 13 Dragons whose names were banished, making it impossible to speak them, making everyone forget them quickly and making it so that when read they are just incomprehensible gibberish. They couldn't even think of their own names anymore.
Another case is the "Unnamed White Dragon". Their name (usual name, not true name) can not be spoken in any language. That's because dragons communicate via telepathy, so the name isn't a word but a series of images and signs. So it just has inherently no pronunciation.
