Anyways, it's really tricky because world "世界 - sekai" refers to either earth or universe due to the system of Japanese logic.
Usually, the main issue why scanners like me or us(?) had problem with this is not just because users in this wiki disagree about "sekai" being the earth or the universe, but because it's not being explained specifically by the author.
There are examples like these but in reverse like Umineko no Naku Koro Ni which already explained every cosmology of it thanks to the storyteller (If I'm wrong, knowledgable members of this verse will probably know this more than the stupid me). And mainly because some authors want to make a very interesting plot instead of focusing at the powers of the characters so they blurred some of the scenes in manga/anime and texts in light novels, that includes movies too.

So, my final conclusion, even with all these gibberish that I said above. It depends how the scenery works in its verse.
If focused on only the question you say, it will make difference when the character said "この世界" -> "This world." If there is some negative words that characters speak something like "この世界を壊してやる！- "I'll destroy this f**king world!" It will constantly states that that character will destroy "Earth." Another if the character said things like "世界を壊してあげよう！ - "Allow me to destroy the world!" It will instantly states that the character will destroy "Universe" and will also slightly depend on character's persona if it's speech is too polite or has much knowledge than others.

Last, another fact is that it will also change the concept of this word if the character spoke it in different places or spaces. If a character spoke these negative words while standing in earth, it's "Earth." However, if the character said a line that has the adjective which translated to "everything", it's "Universe." Moreover, if the character spoke it while flying in space or transformed into large size in space like Sinbad(Magi) and Madoka(Madoka Magica), it's also "Universe." Then it will probably make a difference between about 5-B and about 2-B.
