1. I haven't the foggiest ******* idea and I'm about to head to work, so I'll have to check later.
2. Volo's survival is largely attributed to charismatically disarming his enemies, dumb luck, and having more capable people as allies. In his most recent exploit, he was overcome by a large band of drunken goblins, and only "won" because he sang and danced for them until adventurers showed up.
3. Yeah, depends on the plane. Each has their own effects. Gehenna, for example, just emotion-wipes you into an emotionless husk.
4. Probably. I'd need to look into it, and I'm about to head to work, as mentioned.
5. The Elemental Planes. There's one for each element.
6. Unsure. It's just known that being hit by them doesn't matter. I don't believe it's implicit that they're incorporeal, just that any attack doesn't matter unless it hits them on each plane simultaneously.

I have no idea if the Bard is even in Neverwinter. I've played it precisely once and didn't like it much. As for your earlier questions:

1. It seems the Worm is more accurately a magic item facsimile of a Purple Worm made to punish the denizens of Minauros- the overall lore for its resurrection is, and I quote, "It can be killed, but it always comes back". If we were to assume this is because it is a magic item fueled by something, then the answer would most likely be the Archdevil Mammon.

4. A quick search tells me that the Deep Ethereal (i.e., that portion of it which does not border any proper reality) will passively BFR creatures to a random section of the Border Ethereal (the portion that does border some proper reality) via curtains of vaporous light. This can send creatures to any of the Elemental Planes, the Prime, the Feywild, the Shadowfell, or the Elemental Chaos in which rests other planes such as the Abyss- a character with proper knowledge knows which ones lead where as they are helpfully color-coded. Distance is meaningless in the Deep Ethereal, as is speed as the "space" of the plane functions very differently. No matter the destination, it will always take a random amount of time- the game presents 1-10 hours as standard.

Furthermore, tornado-like events called Ether Cyclones that can sweep through the plane and affect unaware creatures- however, due to the lack of speed meaning anything, the only way to escape is through one of the curtains. If struck, it has a number of effects on the target- a vastly extended journey through the Ethereal, teleportation to a random section of Border Material (similar to the curtains barring the color coding aspect), or being blasted into the Astral Sea (which has its own series of hazards of course).

Huh, but Mammon is banished during the most recent iteration, right? Would his power still fuel the worm's immortality? And is it combat applicable?

Magic items aren't like a direct current to their creator, rather they store their creator's power. That said, to my knowledge, as of 5e Mammon is still in power in Minauros.
